The History of Testosterone Replacement Therapy (TRT)

Testosterone Replacement Therapy (TRT) roots date back to the early 1900s when scientists were beginning to look into hormone treatments. If we fast forward to 1935, and Ernst Laqueur and his team in the Netherlands managed to isolate testosterone.

The 1940s and 1950s: Injectable Testosterone

Doctors started to use injectable testosterone specifically to treat men with low testosterone. This wasn't simply to boost libido. The treatments helped with muscle loss, low energy, and even weak bones. Injectable testosterone enanthate, in particular, became one of the best options to prescribe, because it provided more stable levels over time. As time went on, scientists began to consider whether testosterone might do more than treat low testosterone levels. Could it help with conditions like osteoporosis or muscle wasting? This was the cornerstone of TRT treatments.

The 1950s to 1960s: Pills

By the 1950s, testosterone pills were developed. Unfortunately, the efficacy left a lot to be desired as the liver processed them too quickly. On top of this, there were concerns about potential liver damage. Scientists and researchers then had to consider alternative ways to administer testosterone.

The 1970s: Testosterone Pellets

As part of the ongoing research, the 1970s offered a new solution arrived: testosterone pellets. These small implants were placed just under the skin, typically in the hip or buttock area, and they slowly released testosterone for three to six months. This offered patients an excellent long-term solution, reducing the need for daily pills or administering injections.

The 1980s-1990s: Patches

In the 1980s, testosterone patches were introduced to the market. Patches offered an understandably fantastic opportunity - no injection, no implants, and easy to apply. They offered a non invasive solution for testosterone to work its way into the body.  Whilst they provided a more convenient solution, some men found this to be a skin irritant.

The 1990s-2000s: Gels

In the late 1990s and early 2000s, testosterone gels saw a marked uptake in use. Male patients preferred the ease of daily application, which could be performed at home, with the gels provided a steady release of testosterone. This route became (and still is) a popular choice of treatment, particularly for simplicity and effectiveness.

From 2000s to Today: Constant Refinements

Since the early 2000s, TRT has continued to improve. Long-acting injections like testosterone undecanoate now require fewer doctor visits, and new oral formulations have addressed the old liver concerns. TRT is no longer just for men with hypogonadism—it’s also used for those with age-related testosterone decline.
