What We Assess

Once you have purchased, you will be sent access to a confidential health questionnaire through our secure patient system. It is not a formality. It covers your current health, medications, family history, lifestyle and any specific concerns. Your doctor reads it in full before your appointment, so the conversation starts where it should.
